If you are low income and are enrolled in one of the following programs through CPTC, you may be eligible for additional funding supports and services. The first step in the process is to apply using the process outlined below.

1. Please click Eligibility Application WFD.docx and Individual Employment Plan (IEP).docx and DSHS Consent Form.pdf to download the required application documents.
  1. If you are having problems completing the DSHS Consent form, please download the step-by-step instructions here.
    - An easy way to fill and sign forms is using the free mobile app "Adobe Fill and Sign". Available both on Google Play Store and Apple App Store.
  2. If you are asked to Open or Save, select "SAVE".
2. Open document from where it is saved onto your computer.
  1. Click "Enable Editing" if it appears on the top of the screen.
  2. Do not open in web browser, you will not be able to edit in the web browser.
3. Fill out form in its entirety.
4. Save document.
5. Email document to WFD_Eligibility@cptc.edu
6. You will receive confirmation that the form was delivered to the Workforce Development Team for review.

You can choose to also apply for books and tools during this point in the application process, and send all documents at the same time. To do so, select the "Next" button below, or click the "books" or "tools" link above.
