Kaylani Pangelinan works the second shift on a medical surgical nursing unit that has a maximum capacity of 40 patients. Some days there may be 10 discharges, 6 surgical admissions, and numerous tests and procedures scheduled for the balance of the patients. Kaylani needs to know where the patients are at all times, which patients have been discharged or newly admitted, and which tests have or have not been completed, Kaylani usually needs to reschedule them. If the patient is to have a paracentesis (which is a procedure to remove fluid that has accumulated in the abdominal cavity) Kaylani needs to pull a consent form, make sure the paracentesis tray is on the unit, and communicate the order to the patient's nurse. Once the paracentesis is completed, Kaylani will need to send the fluid down to the lab with the appropriate labels, enter the charges for the paracentesis tray, and call central service to replace the paracentesis tray.
When the unit gets this busy, there is only one way Kaylani can manage it: by staying organized. From the very beginning of her orientation ,organizational skills were stressed to Kaylani. She uses numerous tools to ensure organization, including the following.
At the beginning of her shift, Kaylani receives the report from the first shift. she writes notes regarding expected activities for the patients on the census list next to the appropriate patient name and always add names to the census board as soon as a patient is admitted. Kaylani makes sure that the patient activity board is always nearby, in case someone comes to the desk looking for a patient. The patient activity board can also be used to see if a patient has been taken to a department for particular test during the day shift.
As she transcribes orders, Kaylani knows that it's important for her to update(in pencil so she can erase outdated orders) the patient Kardex so that the nurses are aware of new patient orders- not just exam, but also diets, treatments and activity and observation orders, which can change quickly.
Part of the 2nd shift HUC's routine responsibilities are to graph patient's vitals signs, ensure that patient acuities are entered into the computer, Kaylani makes sure that the patient assignment board is current, check the inventory of unit equipment, and order additional equipment as necessary.
On weekends evenings, Kaylani is expected to record unit statistics, update the on-call schedules, and perform quality assurance on charts to verify that all chart forms are labeled correctly, all orders have been transcribed and allergies are noted in the appropriate places.
